*No one will be denied access to services due to inability to pay; there is a discounted/sliding fee schedule available based on family size and income.
Everyone qualifies for services at STRHS, Inc. STRHS, Inc. provides a sliding scale discount to individuals and families below 200% of the federal poverty level. Patients are not required to participate in the sliding scale discount to receive services. If the patient chooses to apply for a discount the following information must be provided to qualify for a discount:
*Please submit your registration forms and proof of income to our clinic locations. You can access our clinic locations by clicking “Book An Appointment” at the bottom of this page.
- Proof of Income
- Employment Statement OR
- Income tax return (from previous year) OR
- Last (2) Pay Check stubs OR
- Special circumstances may apply allowing declarations

Booking an Appointment
Appointments may be booked at any clinic location.
This is not an emergency facility. For emergencies, please visit your nearest emergency room or call 911.
